CHCDIS018 Facilitate ongoing skills development using a person-centred approach describes the performance outcomes, skills and knowledge required to plan, implement and review formal and informal ongoing skills development, in collaboration with a person with disability, and to incorporate these into the person’s individualised plan.
This Learner Guide describes how to work with the person to identify individual skill development needs. It details how to plan and implement person-centred, ongoing skill development strategies. The content then explains how to evaluate these and review the individualised plan. Lastly, it outlines how to identify and implement incidental learning opportunities to enhance skills development.
The learning content builds knowledge and understanding by using: • Step-by-step instructions on updating individualised plans • Straightforward explanations on monitoring development and providing feedback • Learning Checkpoints at the end of each topic to reinforce positive approaches that are respectful, motivating and empowering • A range of examples and case studies that demonstrate how to use a personcentred approach to facilitate skills development
