CHCDIS012 Support community participation and community inclusion describes the performance outcomes, skills and knowledge required to assist with supporting people with disability in community participation and social inclusion using a person-centred approach.
This Learner Guide explains how to identify opportunities for community participation that suit the needs and preferences of a person with disability. It then sets out how to implement and monitor strategies for community participation and inclusion, which may include supporting the person’s use of assistive technologies. The material also outlines how to identify barriers to inclusion and participation, while providing strategies that help the person address them.
The learning content builds knowledge and understanding by using: • Step-by-step instructions for collaborating with people living with a disability to match their skills and interests to options for participation within the broader community • Straightforward explanations of how to select and monitor options and overcome barriers • Learning Checkpoints at the end of each topic to reinforce the objectives of a person-centered approach • A range of examples and case studies that illustrate how to assist people living with disability find meaningful social inclusion and participation
