CHCCCS004 Assess co-existing needs describes the skills and knowledge required to assess the diverse and multi-faceted needs of people and determine both the internal and external services required to meet those needs.
This Learner Guide describes how to seek additional information from specialists or other sources to prepare for assessment. It explains how to analyse and evaluate the person’s needs using a collaborative approach and how to determine the appropriate services. Lastly, the material outlines how to complete reporting and evaluate assessment and referral processes.
