Observation and Third Party Report

Observation (for the trainer/assessor)

The Observation checklist includes detailed instructions on what tasks and skills the learner should be observed undertaking
The Observation task can be conducted in the classroom, during work placement or in a simulated work environment
The Observation record sheet is completed by the trainer/assessor as the Observation task is completed by the learner

Third Party Report (for the workplace supervisor)

If the trainer/assessor does not observe the learner during work placement, a Third Party Report can be completed by a workplace supervisor
The Third Party report lists details of each task to be demonstrated by the learner during work placement
It outlines important guidelines and instructions to the workplace supervisor on how to complete the Third Party report in the workplace
